Unsafe Work Practices


Hazards is a condition ( unsafe act/unsafe condition)  with the potential to cause harm or injury and it can happen anytime and anywhere. Examples of common WSH hazards includes 

1.Falling from height hazards, 2.Struck by objects,3.Electrical Hazards
4.Machine Hazards, 5.Mechanical Hazards of tools and equipments
6.Fore & Explosion Hazards,7)Noise Hazards,8)Confined Space Hazards
9)Ergonomic Hazards,10)Chemical Hazards,11)Thermal Hazards
12)Radiation Hazards

In the photographs, it was observed that workers are using  their human strength excessively to push the concrete loaded wheel barrow through and fro from the concreting site and they were also observed without wearing any eye protection

The unsafe practices identified are as follows:
1) Workers could suffered from potential body musculoskeletal stress in the long run due to excessively relying on their human strength for conveying wheel barrow loaded with concrete cement to and fro from the work site
2) )Failure to make use of eye protection personal protection equipment

In the photographs, it was observed that  worker is performing torch cutting activity using gloves and eye protections but without the face shield to protect his facial area from blow back and sparks and no fire extinguisher was seen at or nearby the work area

The unsafe practices identified are as follows:
Torch cutting worker is not using face shield to protect himself from blow back, sparks. In addition a  fire extinguisher is not made available at the location for fire emergency purposes. Positive means should be made to confine sparks and slag. Fire watch shall be provided during and after the torch cutting activity to ensure that there is no possibility of fire.

In the photographs, it was observed that one worker is working in a trench with no protective system. That is the workers is not protected from cave-ins by an adequately designed protective system e.g. shoring, shielding, sloping, etc

The unsafe practices identified are as follows:
1)Working in an unsafe condition –that is working in a trench without shoring. Workers must be protected from cave-ins by an adequately designed protective system e.g. shoring, shielding, sloping, etc .
2)Failure of employer to provide safe means of access and egress. Trench shall have stairways or ladders located no more than 25 feet apart. Appropriate testing should be conducted if there is a possibility of an oxygen deficient atmosphere or hazardous atmosphere inside the excavation. Work area should be marked off with appropriate warning signs.

In the photographs, it was observed that one worker was performing cutting/grinding on scaffolding without donning safety harness and another worker was seen working just beneath the  cutting/grinding worker without safety harness and fall protection personal protection equipment and without spark protection

The unsafe practices identified are as follows:
1).Failure to make use of fall protection as personal protection equipment .  In this case  the PPE is determine as safety harness and sparks protection PPE
2)Failure to make use of eye protection as personal protection equipment
3)Failure to ensure fire extinguisher is at cutting/grinding  site for fire emergency purposes

In the photographs, it was observed that one worker was performing painting in an unsafe manner using   a ladder  precariously to access the work area for painting . In addition he was also observed failure to make use of fall protection personal protection equipment. (safety harness) and no barricade was observed being put up at site to deter entry

The unsafe practices identified are as follows:
1)Use of inappropriate tools and equipments as a means to access to an area with height more than 2 meters
2).Failure to make use of fall protection personal protection equipment
3)Unsafe use of ladder – Using a ladder other than what is intended in a way that can threaten his own safety
  at work
