1.1	Are you a trained worker for welding and gas cutting
1.2	Have you put on the correct personal protective equipment meant for welding
1.2) Equipment Set Up & Condition
i	are the manifolds, gas regulators, hose and torches in good conditions?
ii	are the short-length / extension hose properly connected and according to colour coding
iii	are the hosts, joints and valves free from leakage?
iv	are the gas hoses properly arranged or suspended
v	are the approved flash-back arrestor fitted to the gas outlet and inlet of the cutting torch?
vi	are the gas torch and gas hoses provided with valid inspection tags ?
Vii	Are you welding at the designated area meant for welding purposes
Vii	Is there a fire extinguisher nearby the welding location
Viii	Are you trained to operate the fire extinguisher
Viiii	Did you conduct pre site survey to look out for possible nearby flammable substance or ignition source before you commence welding activities
During Welding or Gas Cutting
2.1	a ‘spark-gun’ provided for the lighting of gas torch
2.2	Are the gas regulators set to correct pressure (for gas cylinders only)?
2.3	Is the gas torch shut off before moving around
After Welding or Gas Cutting Activity
3.1	Is the gas turned off?
3.2	Is the supply valve securely closed
3.3	Are the torches and gas hoses disconnected form supply and removed out of the tanks?
3.4	Did you perform fire watch during the lst hour after welding activity completed to look out for possible smoldering smoke
3.5	Did you perform fire watch during the 3rd hour after welding activity completed to look out for possible smoldering smoke
